• Assist in the development, implementation, revision, and reporting of a case management system• Coordinate patient care with the healthcare team• Perform admission assessments and screen for appropriateness• Assist in the development of treatment plans for physical and psychological needs• Coordinate services and discharge planning• Provide patient/family teaching• Verify insurance coverage and obtain authorization for treatment
Requirements
• High School Degree or Equivalent• 3 years of work experience• Experience working with energetic and enthusiastic individuals is preferred
